| The injured employee was part of a crew unloading a semi-trailer of a 6-foot stack of insulated pipe. After cutting one of the four steel bands, the employee stepped onto a nearby 4x4x4 crate in front of the trailer. The employee lost balance, fell and landed in between the tractor and the trailer.
