An employee was using a sewer auger machine to clear a blocked drain at a residence. He had inserted the auger into the outside clean-out when he slipped and fell into a drainage culvert, suffering fractures to the right tibia and fibula. He was hospitalized, requiring surgery.

The injured employee was part of a crew unloading a semi-trailer of a 6-foot stack of insulated pipe. After cutting one of the four steel bands, the employee stepped onto a nearby 4x4x4 crate in front of the trailer. The employee lost balance, fell and landed in between the tractor and the trailer. The employee injured their collar bone and scapula and sustained a cut on the head and a puncture wound on the back.

A truck driver had just dumped dirt from his truck on a shoulder. A skid steer loader began moving the dirt from the shoulder to grade out the slope. The skid steer was backing up to the slope to get its second load of dirt when the truck driver's right arm was caught between the back of the skid steer and the back of the truck. The driver's radial artery and radial nerve were severed and the driver was hospitalized, undergoing surgery.
